Privacy Policy

      

"GDPR" (General Data Protection Regulation) is the most significant piece of privacy and data protection in twenty years. It took effect on 25th May 2018 and from that date we are required to ensure that we gain a new data protection and privacy consent from all clients. In it (amongst other things) we confirm what information we hold about you and how we are permitted to use it.

 

In order to adhere to GDPR you will be required to sign a consent form which will contain the following information.

How we use your information

This explains why Physio-Evolve collects information about you and how that information may be used.

The Therapist who provides care maintains records about your health and any treatment or care you have received from Physio-Evolve. These records help to provide you with the best possible treatment.

Physio-Evolve records may be electronic, on paper, or a mixture of both, and we use a combination of working practices and technology to ensure that your information is kept confidential and secure. Records which Physio-Evolve hold about you may include the following information:

* Details about you, such as address and contact details

* Any contact Physio-Evolve has had with you, such as appointments.

* Notes about your treatment

To ensure you receive the best possible treatment, your records are used to facilitate the treatment you receive.

Access to personal information

You have the right under the Data Protection Act 1998 to request access to view or obtain copies of what information Physio-Evolve holds about you and to have it amended if it is inaccurate. To request this information, you need to do the following:

* Give a written request to Physio-Evolve

* You will need adequate information (for example email address, full name, date of birth and details of your request) so that your identity can be verified and records located.

* We are required to respond within 40 days

If you are still unhappy following review by Physio-Evolve you can complain to the Information Commissioners Office (ICO). www.ico.org.uk

Change of Details

​

It is important that you tell the person treating you if any of your details such as your name or address have changed or if any of your details are incorrect in order for this to be amended. You have the responsibility to inform us of any changes so our records are accurate and up to date for you.
